Employer Identification Number (or Tax Identification Number) – This is a required field entry.

Importer

Name of importer – This is a required field entry.

Phone

Phone number – This is a required field entry.

Address

Street Address – This is a required field entry.  Taxpayers should enter the complete street address including suite numbers, PO box numbers, etc.

Zip

Zip Code – This is a required field entry.  Once the taxpayer enters the Zip code, ExciseWeb will perform a zip code lookup in the United States Postal Database to validate and populate the City & State fields for you.

Tax Payer Ref.

Taxpayer Reference Field – This is an optional entry field.  Taxpayers may use this field for their own internal purposes.  The Excise Office does NOT use this field to process your tax return.

BOL/AWB

Bill of Lading/Air Way Bill – This is a required field entry.  Information should be found on your shipping documents.

Date of Arrival

Importing Carrier
