EIN

Employer Identification Number (or Tax Identification Number) – This is a required field entry.

Importer

Name of importer – This is a required field entry.

Phone

Phone number – This is a required field entry.

Address

Street Address – This is a required field entry.  Taxpayer should enter the complete street address including suite numbers, PO box numbers, etc.

Zip

Zip Code – This is a required field entry.  Once taxpayer enters the Zip code, Excise Web will perform a zip code lookup in the United States Postal Database to validate and populate the City & State fields for you.

City

City – This is a required field entry.

State

State – This is a required field entry.

Tax Payer Ref.

Taxpayer Reference Field – This is an optional entry field.  Taxpayers may use this field for their own internal purposes.  The Excise Office does NOT use this field to process your tax return.

BOL/AWB

Bill of Lading/Air Way Bill – This is a required field entry.  Information should be found on your shipping documents.

Date of Arrival

Date of Arrival - This is a required field entry.  Information should be found on your shipping documents.

Importing Carrier

Importing Carrier - This is a required field entry.  Information should be found on your shipping documents.

Country of Origin

Country of Origin - This is a required field entry.  Information should be found on your shipping documents.

Voyage No.

Voyage Number - This is a required field entry.  Information should be found on your shipping documents.

Port of Entry

Port of Entry - This is a required field entry.  Information should be found on your shipping documents.

Port of Lading

Port of Lading - This is a required field entry.  Information should be found on your shipping documents.

Bond No.

Bond Number – This is an optional entry field.  Taxpayers should only enter valid Bond numbers here.

Customs Entry No.

Customs Entry Number – This is an optional entry field.  This information is found on the Bond if used.
